• This form must be completed by a parent or legal guardian and returned to the youth centre before admission can be permitted.

• I gcásanna tinnis ná taisme nuair is géarghá obráid ná leigheas a thabhairt, tá cead de dhíth ag an Chlub Óige comhairle na dochtúra i láthair a ghlacadh. 

• Cuirtear in iúl do thuismitheoirí/chaomhnóirí má tharlaíonn seo (thuasluaite) go nglacfar páiste chun na hotharlainne agus go gcuirfear iad faoi chúram na hotharlainne. 

• Déanfaidh an Club Óige gach iarracht chun teagmháil a dhéanamh leat i gcásanna éigeandála. Áfach, muna n-éiríonn linn teagmháil a dhéanamh leat ag an am cuirfear páistí faoi chúram fhoireann leighis. 

• In the event of illness or an accident where treatment or an operation is urgently needed the youth centre requires consent for youth leaders to be guided by the doctor in attendance.   

• Parents or legal guardians please note that in the event of the above your child will be removed to hospital or to medical assistance and the youth centre will put your child into the care of the medical practitioner in attendance.   

• The youth centre will endeavour to make contact with you in the event of such an emergency.  However failure to contact you may result in your child being placed in the care of medical staff.
